In NJ : Cigna, Aetna, and Horizon  

In AZ : Cigna, Aetna, BCBS AZ, AHCCCS

Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:

Do I have mental/behavioral health insurance benefits?

• What is my coverage for ASD ( aba therapy)?
• What is my deductible and has it been met?
• What are my in/ out of network benefits?
• How many sessions per year does my health insurance cover?
• What is the coverage amount per day of therapy ?
• Is approval required from my primary care physician?

 

Benefits & Eligibility will be verified with your carrier. 

Payment 

Checks are accepted, payable to ABA STEPS, LLC

Client Forms

Must be completed and terms reviewed / accepted.

Cancellation Policy

At least 8 hours advance notice is requested.
